Updated Toronto COVID-19 Vaccine Criteria

City of Toronto Mass Immunization Clinics:

  • Who is eligible? 
    • Age 60 & older (including turning 60 later in 2021)
    • Age 45 & older (including turning 45 later in 2021) who live in a hot spot postal code
    • Special education workers (regardless of postal code) and all education workers who live or work in a hot spot postal code
      • Must have letter from your school board
    • All pregnant individuals 
    • Licensed child care workers (starting April 29, 2021), must have letter from employer

    • Education workers must call 1-833-943-3900, no online booking
    • Pregnant individuals must call 1-888-999-6488, no online booking

Hospital/Mobile/Pop-Up Immunization Clinics: 

  • Who is eligible? Any of the following, regardless of age unless specified:
    • Frontline health care workers in high/very high/highest risk groups
    • Residents, staff, and essential caregivers in long term care homes, retirement homes, and assisted living
    • First Nations, Métis, and Inuit adults (age 16 and older)
    • Adults receiving chronic home care (will receive a letter if eligible)
    • Faith Leaders (e.g. end of life care, care of deceased, home visits, care in health care and vulnerable settings)
    • Individuals with no fixed address
    • Transplant recipients and those receiving active cancer treatment (excluding individuals receiving solely hormonal or radiation therapy)
    • Individuals with the following health conditions can book in the provincial booking system:
      • Neurological diseases where your breathing may be compromised (e.g. motor neuron disease, myasthenia gravis, multiple sclerosis)
      • Blood or bone marrow cancer diagnosed in past year
      • Very severe kidney disease with eGFR less than 30 (You would have a kidney specialist called a “nephrologist” or may be on dialysis)
      • Obesity (Body mass index greater than 40)
      • Other treatments causing immunosuppression (e.g. chemotherapy, immunity weakening medications – e.g. oral steroids, biologic agents)
      • Intellectual or developmental disabilities (e.g. Down syndrome)
    • You can also book in the provincial booking system if you are:
      • are pregnant (many hospital sites)
      • had an organ transplant at any time in the past
      • had a stem cell transplant (bone marrow transplant) at any time in the past
      • have a neurological disease that makes it difficult to breathe (for example, motor neuron disease, myasthenia gravis, multiple sclerosis)
      • were diagnosed with cancer that affects your blood, bone marrow or lymph nodes (a haematological malignancy) within the last year
      • have severe kidney disease (eGFR under 30)
      • are an essential caregiver for someone with any of these conditions
      • CAMH site only: Diagnosis of a mental illness, substance use disorder, or dementia (currently closed)

  • UHN site only for those who live or receive care in catchment area:
    • Immune deficiencies/autoimmune disorders, Stroke/cerebrovascular disease, Dementia, Diabetes, Liver disease, All other cancers, Respiratory diseases, Spleen problems, Heart disease, Hypertension with end organ damage, mental disorders, Sickle Cell Disease, Thalassemia, Pregnancy, Immunocompromising health conditions & Other disabilities requiring direct support care in community

  • Caregivers (1 caregiver accompanying individual with the health conditions listed above)
  • Age 18+ (regardless of medical history) living in a hot spot postal code
